After passing by Uldum in Southern Tanaris for the umpteenth time I decided to WowWiki it to see what exactly is Uldum all about. Well after reading that theres alot of suspicion on what happened but no actual word I put a few pieces of the puzzle together.
In the WowWiki page for Uldum there is talk about something escaping from it and it possibly originally being one of the labs for the Titans to create things but turned into a prison.
Well they said in the discussion page that it was most likely a snake like creature or a Devisaur (the snake idea from the markings on the pillars). Well that got me thinking, Devisaurs might be likely considering the size and how close the entrance to Un'goro is to Uldum. But would they seriously be strong enough to break free from something that the Titans built?
If you look just infront of Uldum you'll see a Troll ruins. My honest opinion is that it was Hakkar the Soulflayer that was originally there. I mean the size of the hole is accurate enough and he is a serpent-like creature and should be powerful enough to atleast try to defy the Titans. I think the trolls inhabited the now ruined town infront of Uldum and witnessed Hakkar escaping but Hakkar ignored them and took off to hide. They then decided to worship the snake-like creature thus building all sorts of snake statues. The word of the escaped creature got to the other troll tribes who also felt the need to honor the powerful Hakkar. When the Gurubashi got to STV is when Hakkar noticed the trolls worship of him and decided to help the trolls rise to power.
As far as the though of Uldum possibly becoming a high level instance in either WotLK or any other future expansion. Maybe Hakkar wasnt the most powerful thing within there. Maybe he was kept under minimum security while the are more powerful things kept under tighter control.
