Now, since I understand some people may be confused by a few of the options, I will describe them to you.
Name/Gender/Age: I hope you don't have a problem with this, really. Simply, it's your character.
Race: Are you a run-of-the-mill human, a Myrddraal, a Trolloc? How about an Ogier?
Class/Occupation: Whatever your character does for a living. For adventurers, list your specialty (Swordsman, Soldier, Archer, Hunter). For civilians and townspeople, list your occupation (shopkeeper, cleaning an inn, bartending, cleaning the streets... security?).
Nationality: For humans, really. What country was your character born in? A simple, one word answer will suffice (Aiel should tell their clan and sept [optional] here).
Alliance: Who are you aligned to, if anybody? Are you allied with a certain nation, or are you a maverick adventurer? Daring players can even align themselves with the great powers like the White Tower or the Children of the Light.
If you are Aiel, you might as well designate your warrior society here.
Appearance: This should have everything from the character's physical appearance (height, weight, eye/hair color, piercings/tattoos... you name it) to his or her clothing style. And yes, pictures may be used but there still must be a description.
Personality: We want to get inside your character's head. Prove to us that you know the ins and outs of his/her psyche. What would he or she do in a given situation? Give us examples, and a general overview of how your character behaves.
Skills: What can your character do? Anything from channeling to swordplay to cooking should be listed here. If it's not a skill everyone has (i.e. walking or breathing), then list it here.
Weapons: Does your character wield a sword? Does he or she destroy minds with saidin or saidar? Or does s/he not have a weapon at all?
Talents: For channelers only. Does your character have a particular Talent that is rare amongst Aes Sedai? Healing, Dreamwalking, and Compulsion are a few examples of what could be put here.
Biography: Tell us your character's life story. No, you don't have to go into detail about that time he threw up on his mother's apron, or the time she kissed the boy behind the shed, then ran away giggling. An overview will do fine, with a few examples to show us the fine tuning of your character.
PM Color Me Fubar or Inky Winky with this form and name it something along the lines of 'My WoT RP character'... or just the character's name.
