So on the surface would you need a special radiation resistant suit or something?

You could, but the most commonly used protection is Liphith, a liquid chemical that the character drinks to protect their blood from contamination. Its realy the only way to do it, considering wearing a protective suit in the heat of the surface would lead to quick over-heating. So yeah, as long as your character has a few vials of Liphith on them at all times they'll be ok.

Admitfully, even with Liphith, the lifespan of a surface dweller isn't as long as one living in the caves.

what if a volcano or something explodes on the surface causing tsunamis that get into the entrances to the tunnels???

VERY interesting question. Well that is very possable. I never considered that, so there are no flood guards on the entrances of the caves, so that could happen, in qhich case the cave system would be flushed for several miles--depending on how much incline or decline each cave has. The waters are also toxic from radiation, so anything touched would be contaminated and all touched coradoors would need to be quarenteened. There would also be the element of the toxic air drifting into further caves or drifting up into higher caves. The outcome would be total chaos.
