when I started bass I picked up a bad habit of only playing with my middle finger. I say it's bad because I can't play quickly for long using only one finger... I need advice on how I can start to use more (hopefully four or all five would be nice...)

Just practice I guess. Work on using your index, and bringing it in with your middle finger. Concetrate on that, and in a a few weeks to a few months you won't have a problem.

Me i play with 4 fingers
just pratice with one for have a good song and after youcan play with the other finger
it the best methode

mainly cos thats how its done

you have to practise something

its not like...

"hey i only play with one finger, how can i play with more?"

"just drink the Dexterity +5 potion, that will do the trick"

P.S. just read over what i wrote and i'm not trying to mock or offend you in anyway

and i cbf changing it, though i can write more than twice that explaining it

it depends on how YOU want to play. if you want to just use that middle finger you can be like geddy lee who just uses his index finger and strums up and down like a pick.
personally the most comfurtable style is the standard i, and m fingers.
and for practice if you want the standard playing style i suggest doing some chromatic walks with a metranome using just one finger on the right hand. since it sounds like your index finger needs work, focus on that one until they balance in speed, otherwise when your playing blisteringly fast lines one finger will be off beat and eventually throw the other one off as well.
