Traveling underground, there are a roots stretching into the pathway. They all point down the underground road. Following the root-path, a flat wall comes into view. Strange, no? After a bit, the sign comes into view: a red-cross is indented into the wall. This is a hospital, underground! Is it dirty, since it's underground? Not the least bit! The dirt wall outside is the only dirt wall in this building. There's a room to clean off your dirt before you go into actually see the doctor, don't worry. Inside, the walls are made of granite and marble. Also, the staff keep the building sanitary and clean, like any other hospital.
