He just hugged her and held the back of her head with a cutesy face. He didnt relize that she was having a flashback or anything.

While she was out, she dreamed about his death, the killers, the way they killed him, the way they cut her arms, leaving scars and let her live after watching it all and the way Ryan held her before he died. She dremt about it and relived it all.
