Naw, she just didn't work well as an agent because they gave her two AIs. I like to think that they realized that she was going mad, removed her two AIs--who were undamaged--and discharged her. THIS IS JUST WHAT I LIKE TO THINK. It's more likely that she died.
But then again, Wolf, technically speaking Wyoming, South, North, and York are all dead, so...
