I'm leaning toward the first one a bit.
Just so I'm clear, if the blade does not draw blood it will kill its owner?
Perhaps if the knife tries and Alec sees the agony he is going through he may have some sympathy for him?
With him injured pehaps the party may feel they have to take him.

The blade does not kill exactly. It infuses pain through contact. I kind of envision that blade to be one that forever stores up chaotic rage and pain and suffering. Once the blade manages to stab or scratch someone, it infuses all of the built up negative and evil energy into the wounded host, causing them to experience mental and physical anguish.
The build up is faster when she's unsheathed and in battle, but still builds up at a slower rate when she's "asleep" or sheathed. If "alexa" does manage to cut deep enough or stab a vital spot, she will kill like any other blade would in that instance, but she's mainly used as a cruel incapacitating countermeasure against Verans enemies.
If Veran, or whoever is the "host" of the blade cannot or does not injure someone, Alexa releases the pain from the blade into the host itself. She has to have a release and whether that be on the host or their enemy matters little to her.
I'll do the next post and have the blade go after Veran. If you guys take him prisoner, it will make for a very interesting trip to be sure.
