Well Marius has male bits and he uses them. Sirius will do the same for a little while, but eventually she's planning to transition all of the way.
Sirius wouldn't change her name. She likes her name. Despite everything, she's grudgingly got some Black pride. Why else do you think her and Remus have a daughter named Spica? Black family naming tradition.
