I don't think she would be dead. I mean, how could she die? I believe Kushina is still alive, most probably in the Waterfall village, where she comes from.

Unless something came up, or she was captured, for some reason I can't think of. Or maybe she wouldn't be with him to protect him, like how Naruto didn't take his dad's last name so people wouldn't he was the 4th's son.

Pfff, Itachi is the sexiest, by far. Behind him is Gaara, then Sasuke, then Shino, then Neji.
