There are a lot of variables. An important thing to remember is that swordplay doesn't revolve around singular motions. So a perfect attack with a sword can easily be blocked or countered, as is the nature of all attacks, but if the attack leads into another attack as is intended, it has achieved its purpose.
If I step in with my sword across to lock blades for example, in a mock attack, it's a perfect attack if it closes the distance so that I can strike with the pommel. Or if I parry and twist to slash down the opposite side of the defending blade.
Like I said, variables. Even attacks that leave you open for a counter, if they serve their purpose, are perfect. You count on that opening and you get your enemy to exploit it in your favor. Poor warriors look at the immediate picture, whilst great warriors look at it like a chess game. You need to give and take.
