I've been caught masturbating, my mother walked in on me, thank the gods for fast reflexes. She looked at me weird for about a second before she realized what I was doing, she laughed her way back to the kitchen, I've never been more embarrassed.
As for sex, hell, my mom made sure I had a box of condoms and then excused herself from the house.

I've never been caught, but I can tell a story of a couple being caught at a party a few years ago...
They went in the other room to mess around and it might have gone well except for she had a tongue ring and he had a p***s ring... anyway, they got locked together and couldn't get it undone.
Her friend was walking through and tried to ignore the act until she realized why her friend was yelling at her with her mouth, uhm, full. So she tried to help but couldn't get it undone, either and walked in the other room to everyone else to get the guy's best friend...who was heard over all the talking and music yelling WTF, so everyone came in to see what was going on.
First attempt to help was to grab the girlfriend by shoulders and pull her, which got her boyfriend up and yelling to stop. Second attempt was to wait for him to go soft. Third attempt was the best friend to get beside them and get up close and personal to unhook them from each other.
The rings were thrown away pretty quickly.
