I think what the verse is saying is that sin is ultimate death but we who are in Christ will never die. Christ died on the cross for all our sins, past, present and future. He already knows our hearts and minds and what we will do and and we have done. We are already saved and will not be blotted out of the book of life because we confessed that Jesus is Lord over all and saved us. The ultimate sin is when we do not confess that Lord is God and saved us, we remain as a dead one and have not a chance to eteranal life with him. So he is talking to two different people, the ones who have given up their lives for Jesus and the ones who have not.
