The tragedy is that no one seems willing to understand/admit that the poor man is mentally ill. If he'd been treated years ago (rather then tormenting himself with attempts at self-medication/self-healing through surgery and pill abuse), maybe he would be a contributing member of society instead of a pariah with the issues he has today.
Mental Illness was mistaken for eccentricity. The man obviously hated/still hates himself, and had enough money to attempt to make his problems go away. Life doesn't work that way, though.
