HELL YEAH! I have finally found a small amount of info on Fay (and a pic (which i linked):

oh and here's the story about her finally mission:
Setting the Stage
Anakin and Mace are waiting for Obi-Wan's return in the Jedi hangar landing pad on Coruscant. Obi-Wan's fighter is damaged and crash-lands, but both Obi-Wan and the antidote he was carrying survive. As Obi-Wan recovers during med-droid treatment, Mace and Anakin converse. Mace tells Anakin that the antidote is for the chemical weapon used on the Naboo moon. Four Jedi died to get it--all legends: Master Knol Ven'nari, a Bothan called "The Fire Eater"; Jon Antilles, doubtful his real name, was a student of the Dark Woman; Nico Diath, who was part of the Diath clan that produced a great number of Jedi Masters, mainly roamed the outer rim, freeing more than one thousand slaves; Master Fay, an ageless beauty who never carried a saber or used one in combat. All were considered nomads of the Jedi Order, rarely returning to Coruscant.
Obi-Wan and Mace converse in a garden which holds larger than life holos of the four Jedi. Obi-Wan says he hadn't met any of them before this mission and that he'd only heard the stories about them. Mace says, "Not stories, Master Kenobi. Histories. Now, tell us how their histories ended." Obi-Wan explains that his search for the source of the chemical weapon led him to the volcanic planet Queyta, where other Jedi had similarly come to as well.
The Battle
Obi-Wan joins up with the other four Jedi who will compose the strike team. After introductions, they begin their mission to recover an antidote and make their way to a chemical factory that is semi-mobile as it rides the lava flow.
Once inside, they sneak in the shadows of the barely occupied and droid-less factory to find several Skakoans monitoring the systems. As the Jedi begin to access the factory's computer systems, they notice that Asajj Ventress and Durge are also there to remove the antidote from the premises. The Jedi confront the two foes. Obi-Wan engages Ventress with his blade as Durge takes aim at the Skakoans, detonating their pressure suits in a huge explosion. The shockwave blasts the Jedi into the air. They are saved from the flames by Master Ven'nari, who pulls in the fire and disperses it (thus the name "The Fire Eater".) However, this winds up being too much for her and she dies. The explosion has also compromises the facility, which is now beginning to sink.
The Jedi pursue Ventress over a catwalk which crosses part of the lava flow. Kenobi and Antilles block both sides with her in the middle. Ventress says, "You're whole order couldn't destroy me." Antilles responds by saying, "We won't need to. The dark side has already destroyed you. I'm here to release you." "You're all so arrogant..." says Ventress. "...and overconfident," Durge adds as he blasts through the catwalk from below, sending Antilles over the side. "...but you can't fly."
Ventress dashes for the other side, using her saber to slice the ceiling above her as she runs. The ceiling begins to collapse, which knocks Diath into the lava.
Meanwhile, Antilles is still wrestling with Durge from behind, with him in a headlock trying to hold him back so Obi-Wan and Fay can pursue Ventress. Durge says "Go ahead, Obi-Wan! I'll kill your friend, and then I'll find you!" As the two Jedi pursue, Durge grabs Antilles and flips him over his head, pile driving him into the lava. Antilles holds onto Durge's forearms, dragging them into the lava with him. Durge screams, "Aaaagh! Quit struggling, you were dead the moment you met me."
Master Fay, meanwhile, uses the Force to send glass shards and debris at Ventress, bringing Asajj to her knees. Fay uses the Force to enter Ventress's head, while Ventress likewise goes for Fay's heart. Eventually, Ventress collapses unconscious. Fay explains that she hasn't killed her, merely pulled away all of her thoughts. But in another moment, she would have succeeded in crushing her heart. Obi-Wan picks up the antidote vial Ventress was carrying.
As explosions grow closer, Durge arrives, forearm-less. "You're not going anywhere--except the afterlife!" says the bounty hunter.
Another explosion occurs, sending all three in different directions. As Fay gets to her knees, a recovered Ventress walks up behind her and stabs her through the back with her lightsabers. "I'll have your heart, one way or another," says Ventress. Durge then says he's calling in the evac ship, and asks if Ventress is hurt. Ventress then asks where Obi-Wan is.
They find him hanging off a cliff with one hand. Ventress asks if he'd join her and the Confederacy by taking her hand. "Only with my lightsaber", he replies. Ventress tells Obi-Wan, "the next time we meet, I WILL kill you, regardless of Dooku's orders." Ventress and Durge board the evac ship and depart.
Master Fay manages to crawl over to Obi-Wan and help him up. Sadly, the ground is still sinking and the nearest solid ground is over 50 meters away. Obi-Wan says they will have to jump, but Fay thinks it's too far, even for Obi-Wan. Fay grabs his head and gives him what's left of her strength. Obi-Wan objects, but says, "Too late...you will escape...you will live...and I go to join our brothers and sisters...finally." She then collapses.
Obi-Wan continues to speak to Mace, in front of the holo images. He says "She was the strongest of us all, and for a brief moment, I could feel how strong...I suspect she would have lived forever if not for this one sacrifice."
Mace talks about Master Fay saying, "The force kept her alive for many hundreds of years, Obi-Wan. And in that time, she saved many thousands of lives. And, by ensuring you lived, she has saved millions. Let that be some comfort to you." Obi-Wan concludes notes that he won't find peace until Dooku is captured and the Clone War is over. Mace is comforted by that.
Outcome
Knol Ven'nari, Jon Antilles, Nico Diath, and Fay all die in the skirmish with Asajj and Durge. Obi-Wan recovers the antidote to the chemical agent that killed all the Gungans on Ohma-D'un.
