Maximos Squall has a big scar right across his face. It makes him MORE attractive. Just like in DnD it means your scarred, but it ADDS to you. That kinda s**t will all be up to the DM to decide when your take an APP penalty or not

Latent, I don't want you to start feeling like I'm being invasive or offensive here. This is just my opinion, not bashing your efforts. In DnD a lot of disbelief is regularly suspended, and inumberable 'realistic' issues are ignored for the sake of fantasy. The D20 modern system's damage, nearly every bit of DnD's combat/resting/stats. It's usually you either have a high mortality game like Heavy Gear where you accept the s**t as it piles up or you go with the classic style.
Though that might just be my circle of players. =)

Or you could just have certain items give you conditional bonuses.
Like, red hat + red... shirt? = +5 APP
Or red hat + chain mail = -3 APP
Or hideous facial scar + rugged facial hair = +2 APP
Or hideous facial scar + ribbons in hair = -6 APP
The possibilities are endless.

but you will notice that most females never appear more atractive with scars xp
