Actually astronomers are thinking that Black holes could be the enterance to a worm hole (you just get crushed into subatomic particles first)
They have also discovered what they are calling white holes - stuff comes out of them - they could be the exit for these worm holes

They are separate ( tens or hundred of light years ) They are thinking they are the ends of worm holes ( could make interstellar travel possible)
