Wish granted, but the machine is defected and the pinball shoots out of the machine and hits you in the eye, blinding you in one eye.

Wish granted. You become so motivated you can't stop doing things or it'd bottle up. After scribbling on every surface in your home town people in white coats grab you and lock you into asylum. Slowly all the ideas fill your mind and you will become babbling, drooling but extremely motivated person.
I wish I wouldn't need a nose.
