That's a tough one to answer. Some of my hardest challenges are lost to time, since I've played some games so often that they aren't a challenge anymore.
Most of my challenges come from fighting games, where the fastest and twitchiest are the winners...some bosses are just outright cheap. If you don't know the ins and outs of the fighting system, you're just not gona make it, you know? Sometimes, though, I can win through sheer speed and reaction, and those are most certainly my toughest challenges in and out of the game ('cause you're actually tired when you put in that much effort!)
Some games are just challenges in themselves, like Marble Madness, or Ikaruga (or a lot of shooters). So I guess I can't really pick one particular challenge without going through my memory and listing a bunch.

That level in God of War where you have to push the sacrifice up the hill (Of course, I'm not a pro like the guy in that vid. emo ). I almost screamed out loud when I was through with that.

The battle with Ansem in Kingdom Hearts 1 where he is still human shaped. !!! It makes me frustrated thinking about it. It took me at least 25 tries to finally beat him. The rest was surprisingly easy, but at the time I was like "What!?!? He's not dead after all that!?"
So yes... Ansem from KH1 is the single most difficult boss I have ever faced. Before that, it was King K. Rool from Donkey Kong Country. >.<

Believe it or not, I found it harder to GET to King K. Rool than actually beating him. I keep forgetting which zone kept me back for several years until I came back and ate it for lunch.

1) Trying to get over that pink blob in the Great Giana Sisters. 2) Controlling the ball without the power-ups in Wizball 3) Zombies ate my neighbours. The whole game was pretty challenging. 4) Killing Demyx in Kingdom Hearts 2. That guy just wouldn't die.

I once tried playing the old tetris game and when I was in level 21, I think, (yeah there are many versions, but trust me, level is ******** tough in that version) I was beyond hope of winning. That is without any doubt, the hardest game I have ever tried and will ever try.

Rise of the Triad, definitely. Some sort of self-challenge with time limit (for infinite lives), Super Hard Mode on (can be enabled in the config file, as far as I know) and Creezy Man difficulty. Esentially, try to beat ANY level in this game or, what's worse, it's add-on, Extreme ROTT. Without saving. One hit gibs, should only the High Guard spot you, are guaranteed. No warranty on the fact these will be your gibs, though, because from now on, these guys weild "rocket pistols".

Ninja Gaiden 2 was murder. How I ever got to the 7th stage is beyond me. Most of my boss battles were sheer luck, persistence, and down right adrenaline. Sad thing is, I went through all of that, beat 3 out of 4 of the demons, got to stage 7 and then the very first enemies you see rush up to you in a group of around 5 or 6 and one hit you... and it takes a good minute or two of wailing on them and dodging to take down one. I swear, there were so many times when I wanted to break my controller in half with that game.
