If I absolutely had to pick one, it'd be my Zorro Mask. I don't use it at all anymore, but it has been in my inventory since 2003, it was a staple item on my avatar until I went on hiatus in late-2004 and it's most probably the main contributing factor to why my avatar never seems to have a human face.
If I still had my NM scarf from '05, it would have been a strong contender.

My Alruna's Rose because a really great friend gave it to me for a gift last year for Christmas.

Wind Security blanket. Got it on an old account back in like 07 or 08, I don't exactly remember what year it came out, but the october it did, I grbbed it. The account with it, had its trading pass disabled, so I made a new acct, and passed it back to myself. It's the first item of any remote value I got, and It's just such a chill item, I never needed to change my avi.

It's a contest between my reindeer slippers and Horns of the Demon.
My reindeer slippers are the oldest item I have that I earned and that I didn't sell off. I hardly wear them but I'll also never get rid of them.
My Horns of the Demon are the first item I ever seriously quested for. Bought them for 625k in the old MP and will NEVER EVER sell them. I hardly ever make an avi without them.

My Pora. Gold wise, he's easy to get by anyone, but I just find the little guy adorable and keep him by my side at all times. He's my Gaia co-pilot, I got him when EIs were in their infant days and it was so exciting watching what he'd turn into next.

Gogh Reed, I spent months questing it and ended up having someone donating one to me. The fact that someone was generous enough to donate such a big ticket item to me was huge.
It was the first quest I had above 400k also~

'Perfect Tragedy'.
I was at a friends house and still had a Perect Tragedy left when it was still up in the Cash Shop. I bought one with what I had left, and proceeded through the stages. I got up to 5 and decided to end it there. Out of it, I received Jeckyll's Mortality, which I sold for about 1,1 mill.
With it, I've managed to get a bunch of sweet items I always wanted and appeared out of my reach including Alruna's Rose and Death Whisper.
