The crew would be seriously disturbed. They would think he was really a girl and wonder how they didn't notice. Then get a hold of Ace to help them kill the father of the baby. That would be because they're such a loving crew and are fond of their captain and of course Sanji would start flirting mercilessly.
What if Smoker followed the crew to Skypeia?

Luffy and Chopper would miss his stories. Nami would miss someone to boss around that isn't gaga over her. lol Zorro would act like he doesn't care. SAme with Sanji and Robin would just think.
What if Arlong hadn't killed Bellmere and taken Nami hostage?

The Titans and Crew would be scarred for life after Luffy and Starfire became friends and teamed up.
