If you envision it in feudal times than sama means that, but according to class it's used simply whenever you wish to add formality or politeness, or if addressing a superior.
Everyone in the world is better than me, so i use sama according to that final line of logic. unless i wish to add closeness (which implies a sense of attached-ness to a person despite the lack of formality).
