Hmm it was a two heared child in...I think Silent Hill or something. Can't remember what game to be honest....man that was freaky! Also....they aint bosses or anything like that, but the sound the dead people make in FFX man that is freaky!
Also the Redeeds in OoT, I think games just try to freak up out and it never works half the time.

The Skulltulas in OoT freaked me out rightly. It's that... rubbing... scratching noise they make... urrrrgh...
G-MAN - GREAT SCOTT. He is creepy. "I THINK I'LL STAND REALLY CLOSE TO YOUR FACE. Like the crevaces in mine? Ho ho ho..."
Not a creepy villain, but that whole bit in MGS2 when you get those creepy calls... don't wanna spoil, but those who've played it know what I mean.

My creepiest enemies are the Dead Hands from Zelda: OoT. I hated how they dropped down on you from the ceiling and that whooshing noise they made...>___< gave me the creeps.
That, and the Leeches from RE: 0. Not because they were particularly scary, but because I HATE leeches. And giant slimy morphing leeches are even worse.
Oh, and the Insane Cancers from Silent Hill 3.
I scare easily. >___<

I'm in agreence with everyone else- the Redeads from Ocarina of Time were freaky. I kept running into them by accident because I didn't see them *shudder* I also hated the Wallmasters in Oracle of Seasons. They just appeared out of nowhere and took you back to the start of the dungeon. Not only did they freak me out with their sudden appearence, it was really annoying...

Ghouls at Legacy of Kain: Soul Reaver 2. They're mindless goons- but I never feel sure to beat them when I must face them xD

I haven't seen too many creepy enemies (I tend to shy away from that genre of gaming) but there was an enemy in Devil May Cry that made me jump a lot. It could melt into the walls and floor, so that sometimes you just couldn't hit it. Quite often it would come up from beneath you, impaling you on a spike (it was a shape-shifter).

Creepiest was in the PSone game Castlevania Symphony of the night. It was this boss monster thing where everytime you hit it a corpse would fly off and start screaming as it was falling. Then the would wake up and start walking across the screen. It's rather unsettling.

SmilingBomb Those evil doggies from Resident Evil, because when I first played it I was fairly young, 11 or 12, so it was like "OMFG!!! SCARY!" *Goes to lock door* That and some of the FF bosses that you have to beat to a pulp multiple times throughout the games.
