The crew would think he's about to die or is extremely sick for not wanting to eat and Sanji would take it as an insult to his cooking.
What if Ace found out the marines had suceded in capturing Luffy and were going to kill him?

ookay..
what if,
sanji became sick and the crew had to take turns
cooking all the meals until he got better?

lol.
i've been asked this one before.
*thinks back*
well,
it wouldn't be the same without nami.
and they most probably would have never met
gotten sanji (asides from his dream to see the all blue,
he joined to be with nami). they would have never gotten
chopper (they only went to drum island to cure nami,
other wise they would have went straight to arabasta).
all this and without her they'd probably still be wandering the
east blue.
what if..
shanks had let luffy join his crew when he was a kid?

Then he wouldn't be known as Straw Hat Luffy, he'd be Rubber Man Luffy! Not to mention Zoro would be dead and everyone else would be suffering at the hands of various villians.
What if Luffy found out he was actually a prince?

He'd use that to make Sanji feed him whenever he's hungry and say they had to have meat at every meal.
What if Ace had joined Luffy's crew instead of going back to WhiteBeard?

the luffy would have completed the ultimate crew, however white beard would kill ace for going to another pirate crew.

He would have taken his revenge on Robin instead of Blackbeard... then apologize profusely for his mistake and go back to his Blackbeard revenge.

He would still drown and be as cute as ever... and would have more trouble becoming and being a doctor. He'd look really weird after eating the Rumbleball too.
