I've seen this game on other message boards. The game is quite simple. So I'll give two options, and the next person to reply has to pick one and then start one of their own.
Ok I'll go first
Would you rather

Give up smoking. It's supposed to be alot easier than giving up drugs, right?
Would you rather be forced to torture your ex-commander like Sayid or be in Charlies situation with the drugs and family issues before the crash?

I'd say be in Charlies situation with the drugs and family issues before the crash? Just because I don't think I could torture someone.
Would you rather: be stuck in a wheel chair your whole life, or: Have cancer knowing that you were going to die.
This is before the island before Rose and Locke got better.

They are both so horrible gonk but be stuck in a wheel chair your whole life. Still alive this way.
Would you rather be stuck in the hatch pressing the button without being able to go outside or be in the other hatch were you watch the people press the button and write down everything they did?
[{The first is what Desmond did before the plane crashed, and the second is where Locke and Mr.Eko found that hatch, this is what people would have done there or atleast that video said so}]

I think I would pick being Desmond and pressing the button. I can't quite remember if in the other hatch they got to do shifts. But I think i'd rather be Desmond just because he at least got to listen to music and read and stuff.
Would you rather be stuck in the aquarium like Jack in the first ep of season 3
or
Be stuck in that zoo/cage thing like Saywer did In the first ep of season 3.

stuck in the cage like sawyer. cos at least your outside. would you rather have a baby sick on the island with no meds like clair. or would you rather someone take your baby. like the french woman(sorry cant remember her name)

Would you rather try and escape and then run for it with a risk of getting caught or if your cage door was opened and someone told you to run, you'd just stay there, not taking the risk.

I guess I'd take my kid and leave my 4 friends. Just because it would be my only hope to escape. Plus, I'd leave and then tell everyone where we crashed.
Would you rather: Be responsible for Boone's death? Like Locke was in a way, or: Shannons Death like Anna-Lucia.
