Since no one posted I'll start us off. Watching the episode before he jumped I saw beside the building what look like a mattress store. I'm going to say that Sherlock had already deduced that Moriarty would try to force him to jump off the roof when he called Molly. I think he told her what was going on and ask her to get rid of John for him. Later before he jumped I think Molly place a mattress for him to land on. I also noticed that when he jumped the nurses and doctors were immediately there after he jumped. I think they were in on it and helped him look as if he was dead. As for John taking Sherlock's pulse John was most likely in shock, he could have very easily messed up and not felt it. I believe he was later taken to the morgue were he spent the night and in the morning he was let out of the morgue by Molly.

Another theory that I have it that when the bike hit John someone gave him the drug from Baskerville to make him think it was Sherlock's body, when Sherlock really just threw off a body from the morgue.
