I think part of the problem here is that there's a discrepancy between sincerity and politeness at all - which is something you'd have to work on, yourself. Either your definition of "politeness" is so far-reaching that you're actually martyring yourself for those who are genuinely undeserving, or you're actually sincerely antagonistic (or, at best, indifferent) toward others.

Ask not "Why should I be polite if it's insincere?", but "Why am I sincerely impolite?"

It's asking at which point does being polite turn into saying something untrue just to make the person feel good.

For example, someone makes this absolutely repulsive outfit and asks you how it looks, and you say, "Wow, it looks great...!"

In this case, it would highly depend on what others consider polite. If the person asking for your opinion really does consider it polite of you to "tell the truth and criticize them of the outfit" then you should be obligated to do something out of the ordinary and tell the truth.
As Parodius mentions, offer constructive criticism in this case without bashing and mind raping the other person with your "personal comments" if constructive criticism is what they want.
