IMO: I like the character's dark distant side. Will make for variety in the mix! Though there are a few things to fix which you've asked for help. ^^

Main information:

Race: (White,black, Hispanic, Japanese) Ethnicity you'd like your character to be.

Ninja Type: I'm not sure about this but I think you need to pick either 1 or 2 depending if you have a kekkai genkai. We don't everyone too powerful. Plus you're starting in the academy. :3

Main Information:

Kekkai Genkai: What does it do? How does it effect you?

Shinobi Clan: Your Last name. ^^

Current Bloodline stage: Means whatever your Kekkai genkai does how many stages before you reach the final? Take Uchiha for an example.

Chakra Element: Has to be out of the ones listed on the post Kurtis made to be fair. And you can only chose 1-2.

Battle Weakness: You have to have something. =/ Everyone isn't flawless.

Shinbi Skills: Top 3 you are good at. Speed? Close quarters combat? Long range combat? Stealth? Endurance? Strength?

Complete Missions: Since you're beginning at the academy I don't believe there should be any missions listed. It's not until you become Genin you get missions? Might want to ask Kurtis just in case.

Headband: (For Kurtis too) If starting int he Academy they don't have one, they have to graduate to get one so do they leave it blank and fill in later or just leave it filled in?

Technique List:

You start with the first three automatically for academy, not sure about the rest. I think you're suppose to pick from the lists that are provided by Kurtis. I'm not sure how he feels about made up jutsu's so again this would need to be double checked with Kurtis. And depending what 1-2 elements you chose is what jutsu you can do.
