This is a great example of what you could use. I snagged it from my friend.
The following is a list of the information that you have to have in your character`s profile. You are not limited to just this, however; in fact, the more detail you write into your profile, the more I am inclined to accept it.
✖ Character picture(s), with excess pictures in link form.
✖ Full name and nicknames.
✖ Character position.
✖ Age, gender and sexuality.
✖ Likes, dislikes, and fears; list or paragraph form.
✖ The classes your character will have in school.
✖ There is no specific list of what classes the school has,
✖ so just think of whatever classes a high school would have.
✖ I suggest that you place your character in as many classes
✖ with other characters as possible.
✖ Biography, at least a paragraph.
✖ Personality, at least a paragraph.
✖ Posting color(s) and username.
