Each uchiha clan successor is the eldest son. When Konoha was formed Madara was the clan leader 64 years ago (current time line) and for itachi to be the clan successor and his father fugaku to be the old clan leader they'd have to be madara's direct descendants. Fugaku though is too young to be Madara's son seeing how Madara would be in his late 60s when fugaku was born so it'd be Fugaku's father who'd be Madara's son. Thus, making Sasuke and Itachi Madara's great grandkids.
Another fact:
Madara: Would of been about 39 when konoha was founded and about 53 in the fist ninja war and if he fought in the second and third he'd be 71 and 86 roughly.
Madara's Son: Would of been about 12 when konoha was founded and was about 26 in the first ninja war and 44 in the second but would of been dead or too old to fight in the third.
Fugaku: Would of been about 17 in the second war and 32 in the third.

iKoisu Or Cousins... they could be cousins... Most likely they are a direct line if it goes to the eldest child because itachi was heir to become the clan leader just like hinata is heir to the hyuga clan because she was the oldest child and her father was the oldest child.

Uhm, Neji is the heir. Hinata is from the Sub-Branch. Just like it is possible for Sasuke and Itachi to be from the Sub-Branch...
