i never buy books twice by accident (unlike my Dad) unless:

- i lost a book. this happened with Carpe Jugulum by Terry Pratchett which i couldn't find for about two years so i bought another. after which it was promptly found at Grandma's gonk

- to complete a series "properly". i've become quite pernickity about this as the fact that all of my Skuldugery Pleasant series are 1st edition except the first one REALLY annoys me so i want a 1st edition version of this.

Yes. I bought:
Harry Potter and the Philosophers Stone
Harry Potter and the Chamber of Secrets
Harry Potter and the Giblet of Fire
Harry Potter and the Order of the Phoenix
Harry Potter and the Half-Blood Prince

I couldn't find my hard copies of them. You see, my mom started reading them after we saw the movie and because I was young, she read them quicker and so by the time she was done with the first 2 the third was in hardback for christmas and then we got the fourth in hardback from my cousin and then we bought the fifth in hardback, then the sixth when it came out, then the seventh. The only one I could find was the seventh. So I had to buy all of them again. Two months after I finished the seventh one, I found all of them.
I had a year period when I stopped reading the third. I had that one already so I didn't need to look for it.

I have never bought the same book twice but once for christmas my mom gave me a book and when I went to celebrate Christmas at my dad's house he gave me the same book. The good thing about that was that my friend needed to borrow that book and I was reading it so we could both read it at the same time.

Arggh.... I get so excited when I see a book for a series I like on sale and I buy it to put towards my collection only to come home and find out I've already bought it! Hence, I plan on working on a database of my books... there are some great programs on the internet... However, the process could take a few years given the number of books I have... I'm like the Paper Sisters- I have books in the attic, books in my closet, books on my shelves, books in my POD storage bay and books in my parents' storage bay and books on the downstairs shelves...

Yes I have if it's one I want to read again later yet threw it away. Or in the case of the Stand I bought it twice because I wanted to read the "compete version"

I've had duplicate copies fall into my lap after I bought a book.

I'm so, so tempted to buy two or more copies of Good Omens, that way I could have the black and white versions. The only thing that's stopping me is that the first one I bought was a mass market. I really want the quality paperback versions. I suppose I could always buy both of them, but at $14 each it seems to be a bit much.

Otherwise, my family bought three copies of the last few Harry Potter books so that we could all read it the day it came out. xp

I'm probably going to purchase another set of The Lord of the Rings, since my current one really belongs more to my family than just me and it's rather worn down.

On accident? Oh yes. I got two copies of Valentine by Tom Savage, one at a book fair and one in a stack of other books.

I had two copies of Interview with a Vampire and Queen of the Damned because I bought one copy of each on their own and then the whole series - matching covers for looks - from eBay.

Then on other incidents (rare, but it happens) I'll buy two of a book because it belongs in a series and I need to read it before the paperback comes out, or I buy the paperback when the rest of the series I have is in hard cover.
