Hi fellow Sherlockians biggrin I believe he landed on a truck we can see for a few frames on the scene, the truck seems to be loaded with some type of sheets. We know he went to Molly for her and she works in the morgue. As we know the Morgue is used for the storage of human corpses, I believe he might have asked Molly to lend him one of the bodies that looked the most similar to him and that is the one we see on the street. That way the other doctor in the scene can be fooled as well. Since they were in front of an hospital. Now the question is how did he fool Dr. Watson? others can be fooled as they have only seem him on newspapers but not Watson How is the boy in the bike involved, is he important somehow?

I think the boy on the bike stabbed him with a injectable form of the Baskerville's drug so he wouldn't know it wasn't Sherlock.

I'd like to give my idea but being in the UK I've already seen the episode so I can't because I know what happens and it'd be kinda pointless... But before I saw the episode I had generally the same theory as most of you. I'm not telling you what I thought after I saw the episode because that would ruin it for you guys.
