It'd be hard to out-do the Otokage in terms of punishments...
Hell, Shuniji's "reward" for being first of his team to complete the mission (by running back to the village at the first sign of danger) was the privilege of cannibalizing a member of the group they were supposed to investigate.
Kaichou was late to the Otokage's chambers. He had all of three speaking posts in the guild before being mentally tortured, tossed in a casket, stabbed repeatedly and had his mouth sewn permanently shut and his lungs filled with a mystery liquid.
Shuniji lost in the third round of the Chunin Exam to Ichi. He was forced to wear a mask until he killed Ichi, but ended up getting it stolen after he was abducted by a cult.
The most recent punishment was because he was abducted. He was set on fire by Amaterasu, and now due to the skin grafts, he'll probably look like a mummy until next season.
Seika got off easy with the invasive snake surveillance that isn't even there now.
Oh, right, and we all had heart seals that would cause our insides to explode if we were ever away from the village for too long. Shun's the only one who still has one.
So, yeah. Mizukage's a badass, but the Otokage is admittedly more creative in his terror. sweatdrop

...I'm happy Akeru isn't a mist ninja, she definitely would have gotten herself executed already.
And if she was a sound ninja...I don't even want to think of what the Otokage would do to her.
Having a character that likes to cause trouble is dangerous to said character's health... sweatdrop
