5k for shoulders up
10k for waist up
One Sealed Letter or Monthly Item for full body
Multiple that by two for couple pictures. (Or three for triples, etc.)
Colored is 5k extra.
I'm pretty flexible when it comes to accepting items, especially Cash Shop items, so if you have extras you'd like to pay with, it can't hurt to offer.
If you want a general idea of what it'll get you, just check what they're going for in the Marketplace. If it's selling for around 10k, you'll probably get a waist up for, for example.
Payment:
I used to be pretty flexible on this, but now I'm ask people start the trade before I send them the picture. If you want to wait until I PM you to tell you I have finished the art, that's OK, but I've just had too much trouble getting people to pay. (One person claims to have paid, another is just ignoring me.)
Notes:

For avatars, either keep your avatar dressed as you want it until I finish, or post a picture of how you want it in your post, so I can work from that.
For Original Characters, please include full physical descriptions, picture references appreciated.
I prefer non-cluttered avatars. ^^;;
For something expensive like popular Evolving Items or other very expensive items, I can do big group shots in colors or something... PM me, we'll talk. ^_^
