Okay, it's not so much different from a food shop, but this is generally what you do.
1- Fill out the form (which you did perfectly)
2- Send me a trade with the total amount of gold. I can't start working on your order until it is sent.
3- When I get you trade, I'll send your order to you through a pm (make sure it's set to receive messages from everyone).
4- You will have to save your order to your computer or to an image server like tinypic or photobucket. To save to your computer, just right click and pick 'save as'. To save to a server, right click, pick 'copy image location', go to your account, pick upload picture (there should be an option to switch the source from your computer to a source from the web), copy and paste the image location into the box, and press upload. And fini!
