Perfect Tragedy Forfeit Item List (TELL US WHAT YOU GET!)

Usually in RIGs with different stages like the Perfect Tragedy, someone makes an item list that has the specific items you can get at each stage when you forfeit/exit/etc. I haven't seen one for the PT yet, so I decided I'd make one. Here's how you can help out: Play some RIGs and instead of going all the way to the Encore, forfeit at some stages and see what you get. I've heard of people getting pretty good items from quitting at around stages 4, 5, and 6, but I've yet to see many specifics. This could help lots of people who are after certain items, so let's get going! Onto the list~!
