Think of it like Scarecrow's drug from Batman Begins. It's designed to make everything around you look freaky and scary as holy hell. Darren will be donning a mask meant to freak the hell out of them as soon as they see it. So...yeah...a bad LSD trip. It'll take time though. After the gunshot though, he'll be giving random people much faster acting doses. Like any drug, if you put it directly into your blood stream, it works a lot faster.
