I use the brush tool. I've tried using vectors, lines, and the pen, but I enjoy the manual drawing. Occasionally I'll use MS PAINT, which is a fun program.

Practice, I suppose.
Although, my hands aren't very steady as well, so I make quick strokes to make them appear smoother.

Also, with the photoshop program that I use, there's a setting where you are able to adjust how sensitive the brush is. It won't pick up all the "jitter" that your hand has naturally, or, you can change it to pick up even the slightest movement.

i only have a bamboo tablet but i really want a wacom cintiq, but its too expensive

I use the Ms Paint program too. I use the lines to help me get the picture straight.
