I just throw a name on and if it doesn't stick, I make a note that so-and-so is now named such-and-such, and keep going.

I'm lame and not making up names though. My main character is named Frank. lol I am bastion of creativity.

I usually make either references to someone that fit the story (i.E. Davy Jackson = Davy Crocket + Andrew Jackson) or I just name them after people I know.

I also have a tendency to name characters after people I don't like.
That's always amusing. xD

I usually develop personalities first, the name comes to me once the character is all fleshed out. Names for me generally reflect a character's personality or abilities. As for last names, those can be a bit trickier. I try to match up first and last names that sound good together.
