She could have just forgotten. What's the worst thing that could happen if she doesn't accept? You'd be in the same boat you are now.

That's what I think. If she didn't write it down it's possible she had forgotten the last name by the time she the chance to facebook.

You have nothing too loose by trying to add her.

Don't try add her = don't be friends
Try to add her and get rejected = don't be friends
Add, and she accepts = maybe friends.
