Help a commissions newbie out?

Does any one have a commission template or a sample commission I could use, or just general advice about commissions? I want to create my own commission thread and was just wondering if there was any thing I needed to know.

If I do get a good answer or template, I guess this thread could be used as a Q and A or a FAQ for newb's like me.
