I'll say it, but it's a spoiler, so I'll put it in white. Izumi coughs up blood because when she tried to bring her dead son(Wrath.) back to life she gave up half her organs in the transmutation.

[Hawkeye] I was upset when I first found out... Me too....i felt so bad. Losing any organ is worse than losing an arm or leg or both.

Do not read this unless you want to know why she pukes up blood. It's a major spoiler!
This is all done by my bad memory...
Her baby was deformed and was about to die, but she tried to revive it. Later she goes to the gate and she gave up her inner stomach organs. The gate takes her organs for exquivalent exchange, but of course, it was a failure. So that's why she constantly pukes blood. Thus Wrath was born. That's why she tried to protecy him from Ed and Al.
