ORDER FORM

If you wish for me to make you a layout, please fill out the order form and post it. You may choose multiple items from each fill-out, but for colors please put them in the order you would like on the finished product. Top Line is 25g, Your Text is free, Bottom Line is 25g, each Picture is 20g. Once I have recieved it and it has been started, I will quote your order. When I am finished I will post it with your username in bold letters on top. In order to recieve layout simply quote it and copy the layout under the words copy below. Test it in this forum once to make sure everything is in order. If you are satisified, quote yourself and say so. I will send the trade.
