Her original name was Ditzy Doo. The fandom named her Derpy Hooves and it stuck. During "The Last Round Up" she was called Derpy by Rainbow Dash but that kind of back fired. People said that it was offensive to call her that. (I still think more of the people who said that were people who didn't like the show or over sensitive mothers)

Hasbro has even dubbed out the name "Derpy" in the episode. And were planning on getting rid of Derpy (Ditzy) all together. Which is why she wasn't seen in season 3 other then the season 3 final. (I have hope that they aren't getting rid of her as she was in the movie and is in the CI)

Then you missed it, there was group who suddenly got offended after the last roundup aired over derpy and there was this whole deal about it for a while. I think that’s when he name and voice were changed

I will never understand the fan indignation that ALWAYS follows this character. It doesn't matter where it is or what it is--if this gray pegasus is involved, someone is going to be indignant about something.

Derpy Hooves has long been understood to be a fan name or a nickname for the character. There is a lot of conflicting information about what this pony's real name is--probably because she was never meant to be named in the first place and still doesn't have an official name, from what I understand. I know the name Derpy was also adopted in studio but to avoid issues with the fact it is considered offensive to some people, it seems really unlikely Hasbro would actually have her officially named that.

Ditzy Doo WAS supposed to be the official name of the character. There are lots of sources that explain this. The nickname Derpy was given to her by fans and she was apparently called this, in the show, as a nod to her fans but apparently, officially, she is still unnamed.

her name has not "always been" ditzy doo because she was originally an unnamed background pony. "ditzy doo" was kicked around as a possible name for her because it was mentioned in an episode describing a similarly scatterbrained pony. her name was, at one point, derpy hooves per the show's creator:
