Kaoru is the younger twin brother of Hikaru Hitachiin, though by how long is not known. He, and Hikaru are both in their first year and classmates of Haruhi Fujioka. They tend to imply an incestuous (or "brotherly love") relationship for their "boy love" fans. Both of the twins address Tamaki as 'my/our lord' instead of 'king,' due to the fact they first thought of Tamaki as 'the idiot of lords.'
Both twins felt Haruhi's presence in the club was questionable at first. She could tell both twins apart with ease. To them, it was the first time that anyone had ever stepped into their world. Before Haruhi and the Host Club entered their lives, the twins thought of the world in their lives as 'our world' or outsiders as 'everything outside our world' because they never had any personal relationships with anyone else other than themselves.
As Haruhi points out, Kaoru is not as "mean" as Hikaru. Kaoru is more thoughtful and considerate of the feelings of others. Otherwise, Kaoru likes the same things as Hikaru, such as maple syrup, Italian and spicy foods.
In the anime, it seems Kaoru wears orange apparel when outside school grounds. Examples of this can be seen in episodes 7, 8, and 19. Kaoru and Haruhi are currently close friends.
In the manga, both twins like Haruhi. Only Hunny and Mori know of Kaoru's crush on Haruhi at the moment.
