Heeeeeeere we go. xd Normally when I have to use creative comma arrangements, the grammar tends to be questionable and basically unfixable without recasting and breaking the guidelines I'm supposed to follow. I found an old one I'd saved that amused me: "I discussed this with Dr. Smith at 1945 hours since if we did find, for instance, that he had ischemia possibly due to an embolus whether we would anticoagulate him given his previous gastrointestinal bleed, and Dr. Smith stated that yes, he thought it was reasonable to do the CT and MRI." whee

Also things like "prostate enlargement with symptomatic benign prostatic hypertrophy with large prostate." So, you mean to say... "Large prostate, with large prostate that's showing symptoms of being large, with large prostate?" rofl

I dunno, maybe I'm just a snarky snark, though. I can never completely rule that out. xd

I've had to train myself to read things in a primarily functional way, since the goal in a medical record is usually to make the important features stand out visually so the info's easy to find at a glance. It's also a lot faster to edit a sentence when I don't have to slow down and read it twice to verify that I understood all the sub-phrases (this is nice because we're always encouraged to speed up and "proofread as we go." I've noticed that some of the sentence structure and awkward phrasing I work with has started to seep into the way I talk/write. xd Been kinda curious about whether the comma versus parentheses issue was a common thing among grammar nerds or if it was just a result of the formulaity of my job.. couldn't remember if it always bugged me or if it started in the last few years, LOL. This pet peeve of mine surprises me quite a bit, actually, because I have always avoided anything resembling math whenever possible. User Image

Do you ever find yourself in the position of having to edit something for grammar while changing only the bare minimum (i.e., can't recast unless absolutely necessary for the function of the sentence, like subject/verb issues and such)? And does it make you feel like smashing a brick wall with your red pen? User Image I sit here all day wishing I could apply liberal, gratuitous use of parentheses as if the sentences were all math problems (we're not really supposed to add parentheses, either User Image ). We basically have to abuse commas and semicolons until the sentence just looks like a mess on paper. gonk

"Patient just finished having a certain skin condition with a really long name, where the differential included a more specific skin condition with a really long name and yet another skin condition with a slightly shorter name, excised by Plastic Surgery."

Pretty tame example of it there, but with so many words in there it's so much faster to read the actual sentence if that extra info about the differential is clearly indicated as parenthetical. User Image
