Actually, I do find it reasonable to think that both cannot occur together. The latter (refusal of treatment) is certainly not uncommon. But "happy to have cancer" is a wild stretch of imagination, better left to the tabloids. To clear up a misconception here: I'm opposing the notion put forth by OP that all gender dysphorics should be put on a regimen of anti-psychotics and conversion therapy. If I seem otherwise, it's not my intent.
