Personally I find Dean's "Are you Okay?" to be perfectly in-character. As an oldest sibling who does actually feel pretty close to my younger siblings, I can totally relate to this. I agree that I would have liked more of a reaction from Dean, but at the same time, there's Sam. Cas is awesome, but Sam outranks Cas in Dean's heart. You could see how tempted he was to run after Sam, but he didn't, because Cas needed him in that moment too. I think Dean was running on auto-pilot after that, because he'll save Cas but that doesn't mean he's not freaking out about Sam. ...
