With the intestine thing, I'd say just better slitting open a belly and pulling some out. Easier to get to, and since there'd be less pain (depending on how you did it), you could keep them alive for longer... in fact, with a sharp enough knife, they might not even notice the cut for a while...

Pertaining to the skull, I think that it MIGHT be possible, but they'd have to be on some heavy stimulants, or just flat out hopped up on adrenaline... which they might be, depending on how crazed they are...

However, you'd have to crack open the skull pretty damn hard to have a hole big enough to scoop brains out. Cracking the skull, no problem. Cracking it OPEN, that would be more difficult.
