I noticed a flaw in a lot of writing and especially in manga. Writers have no separation between what a character thinks, what a character says, and the character's actions. They are honest in every aspect, and it makes for unrealistic characters.
How is that a flaw? It's true to life.
((That might be a cultural thing--tons of connotations get lost in translation. Japanese characters seem to be more straightforward, blunt and serious than Western ones, which is pretty much in line with how people from Japan tend to act. But if I remember correctly, they also have concepts like "public face/private face," so dishonesty is also part of the culture. I don't claim to understand Japan well.
Then again, it could just be due to bad writing or bad translations (where they just don't bother to make the thoughts and dialogue of a character sound different). But in defense of this, the "thoughts" are usually past-tense first-person narration, so it makes more sense if you think of it that way.
